I, , do hereby agree to allow the Law firm of Albin Oldner, Sky Investigations, LLC, and/or its affiliates to work with my insurance company, , for the sole purpose of obtaining information regarding Southern Claims & Restoration, Inc. (SCR, Inc.).  This information should include the final invoice submitted, the contractor worksheet or statement of work, any faxes and or emails between the stated insurance company and Southern Claims & Restoration, Inc.  (SCR, Inc.).  Also included should be the adjusters worksheet and/or statement of work, as well as any supplements to the claim.

This is NOT an effort to obtain any Personal Identifiable Information such as social security number information, but is ONLY an effort to obtain the above mentioned information in order to prove or disprove fraudulent activity committed by Southern Claims & Restoration, Inc. (SCR, Inc.)
